40 year old male patient with right hip pain

Findings
Coronal STIR image(1) and sagittal STIR image(3) show hyperintense signal in the right femoral head, neck and intertrochanteric region. Corresponding coronal T1 image(2) shows hypointense signal in the similar region while the axial T2 image(4) shows no significant hypointensity/serpiginous lesion. Mild synovial effusion of the right hip joint also observed with no significant articular surface erosions.
Diagnosis
Transient osteoporosis of hip. Differentials – stress fracture and septic arthritis.
Summary
Marrow edema involving the proximal femur with no signs of avascular necrosis or arthritis suggest transient osteoporosis of hip.
