Findings

1. Knee Radiograph showing lytic lesion involving the proximal diametaphysis of the left tibia with thin incomplete fracture line seen through its anterior aspect.
2. Sagittal PDFS MRI shows septated hyperintense lesion involving the proximal tibia that shows T1 iso/hyperintense contents(image 3) suggestive of hemorrhagic components. Sagittal T2(4) shows fluid-hematocrit levels in the components of the lesion.

Diagnosis

Aneurysmal bone cyst.

Summary

MRI is the best imaging modality in diagnosis of majority of the bone tumors. Correlation with Radiographs and CT scans aid in a confident diagnosis. For bone lesions, multi-modality approach is imperative to reach a diagnosis. In this case, MRI clinched the diagnosis.
