36 year old patient with lower abdominal swelling and cyclical pain

Findings

Axial T1 images(1 and 2) show T1 hyperintense lesions involving the bilateral aspects of the lower abdominal anterior abdominal wall. They show heterogeneous T2 hyperintense signal intensity in the axial T2 image(3). In the sagittal T2 image, LSCS scar is noted and the lesion is seen adjacent to the cutaneous scar.

Diagnosis

Scar site endometriosis.

Summary

In post-operative patient with abdominal wall lesions with hemorrhagic signal and cyclical pain – endometriosis should be suspected.
