Findings

Axial, coronal reformatted, 3D volume rendered non-contrast and post-contrast excretory phase images of KUB region reveal multiple small hypodense lesions in bilateral renal sinuses along pelvicalyceal system. The renal sinus lesions can be seen separately from the contrast filled pelvicalyceal system in excretory phase images. Left lower ureteric calculus causing mild left hydroureteronephrosis is noted in volume rendered images.

Summary

Peripelvic cysts are asymptomatic cysts that arise from the lymphatics of the renal hilum. They usually are multiple, small, and bilateral. Their appearance is often difficult to distinguish from hydronephrosis on ultrasonography and non-contrast CT scan. However, the distinction is readily apparent on contrast-enhanced excretory phase CT images, where the high-attenuation contrast agent–filled collecting system can easily be distinguished from fluid-attenuation renal sinus cysts.
