Findings
Sagittal T2(1), sagittal T1(2) and sagittal post-contrast T1-FS(3) show signal alteration in the anterior and posterior corners of the visualized vertebral bodies with enhancement. Coronal STIR(4) shows ankylosis of the bilateral sacroiliac joints with no significant marrow edema.
Diagnosis
Inflammatory spondyloarthropathy likely Ankylosing Spondylitis. Later diagnosed with Ankylosing Spondylitis clinically.
Summary
In young patients(predominantly males), if we see vertebral signal corner changes(even if subtle), it is imperative to have a look at the sacroiliac joints. If findings are atypical, HLA-B27 should be suggested.
