Findings
- Axial PDFS image(left) shows avulsion injury of the medial pole of patella with defect and marked fraying of the medial aspect of medial patellofemoral retinaculum suggestive of complete thickness tear. Lateral translation of patella is also noted.
- Axial PDFS image(right) shows small PDFS hyperintensity in the lateral femoral condyle suggestive of impact contusion. Marked knee joint synovial effusion also noted.
Diagnosis
Traumatic medial patellofemoral retinaculum tear with patellar dislocation
Summary
Impact contusions are important tell-tale signs of ligamental injury. Even if there is no frank tear of medial patellofemoral retinaculum as seen in this case, it should be looked for in case of impact contusions of medial pole of patella and lateral femoral condyle. One should also look for other causes of transient patellar dislocation.
