55 year old gentleman with left sided vision loss

left sided vision loss

Findings

Coronal T2 fat-sat image(1) shows hyperintense signal of the left optic nerve that is confirmed in the axial T2 fat-sat image(2). The right optic nerve is normal in signal intensity and so is the optic chiasma(arrow in 3). Axial FLAIR(4) shows cerebral atrophy with mild signal alteration in the bilateral periventricular white matter suggestive of ependymitis granularis/chronic microangiopathic changes. No isolated lesions were seen in the cerebral parenchyma.

Diagnosis

Left optic neuritis.
