Findings

Chest Radiograph shows increased cardiothoracic ratio with flask like configuration of the heart. CT scan demonstrates pericardial effusion with pericardial thickening and enhancement.

Summary

Pericardial effusion is an important differential in cases of increased cardiothoracic ratio, apart from Cardiac disease per se. Chest Radiograph, in modern days, is not the sole modality for diagnosis of Cardiac conditions, but knowledge of signs of pericardial effusion is imperative for accurate diagnosis.
