Findings

Chest Radiograph(1) shows confluent upper lobe dominant opacities with calcification foci and associated fibrotic bands. Bilateral lower zones show lucency likely due to compensatory hyperinflation. Axial NCCT(2 and 3) show calcified lesions involving the bilateral upper and right middle lobes with calcified mediastinal lymph nodes. Axial NCCT of upper abdomen(4) shows calcified lymph nodes with multiple calcification foci of the spleen(we can see the calcified lymph nodes and splenic calcification retrospectively in the radiograph). Axial lung window(5) shows confluent fibrotic areas with associated fibrotic bands with upper lobe dominance(correlated in coronal image-6). Fibrosis and compensatory hyperinflation of the bilateral lower lobes also noted.

Diagnosis

Silicosis – patient was a rock mine worker since 15 years. Had no history of past tubercular infection.
